This notice covers this website only. Your medical records and protected health information are governed by our Notice of Privacy Practices, which you receive as part of the new patient packet.

What we collect

When you send an appointment request, we receive the name, email, phone number, and message you enter. The form is delivered to our office by email through Web3Forms. We use it only to respond to you and schedule care.

Please do not include personal medical information in the form or in email. We will discuss your health with you directly by phone or in the clinic.

What we don’t do

This website does not use advertising trackers, and we never sell or share your information for marketing. Links to the athenahealth patient portal, Fullscript, and Google Maps are governed by those services’ own privacy policies.
